Just me or is meal plan super confusing

So trying to decide between the plus and the premium.
I understand you will only get the value of the premium at a set number of restaurants.
But where they lose me is they say Plus is "set menu" and Premium is "a' la carte".
But they never define what a' la carte means and what you can order. In Disney World they will tell you, Drink - Entree - Dessert.

On the surface if it was app, entree and dessert and looking at the menu and being able to choose anything it seems like a great deal.

But as I look at the menus, in addition to starters, entrees and desserts they have super confusing "Set Menus" named cutesy names. Cowboy menu or Tinkerbell Menu, or Walt's menu. But on top of they use Signature and Premium on some.

So with Premium can we order off the menu or only from the Set Menu of Premium Selections?
