We just returned from WDW 2/13-2/20 and used our vouchers for fastpasses at each park except Epcot. At AK we had 3 from Donald group (EE and safari) and 3 from Daisy (Kali River, Tough to be a Bug and Dinosaur). We used 2 for EE and 1 for Safari; kids used 2 for Dinosaur.
We felt that at MK we wasted our voucher for the FPs (this was on Sat 2/13!) because we could easily get on everything and collected regular FPs. We opted not to use it at Epcot because I thought the same would be true - and we were fine without it.
However on our DHS day (Tues 2/16 - a Fantasmic day) and AK day (sat 2/20) the FPs were worth their weight in gold! At DHS the regular FP machines were down and CMs were giving out FPs - we ended up with more than we could use in 1 day and gave some away.
It is so wonderful to walk by a line that has a 130 minute wait time and say - let's get an instant FP and ride it now. Worked like a charm!

Just to be sure you know, there is no Birthday promotion. This year it is called "Give A Day, Get A Disney Day" and you need to volunteer a day at a qualifying organization...
It started the year at 2. Then it jumped to 4, and then around a bit. I don't think it was ever certain that it was due to crowd levels, at least as the sole factor. Different parks could have different numbers on the same day.
It was typically set for a week or more at a time, not a daily basis.
Towards the end, 3 seemed to be the common number, and so far it has been as well, at all parks. It could be that they decided 3 is the sweet spot. However, if the FP use rate is higher than last time (and I'm not sure it will be - a lot of people can take advantage of the ticket value instead this time), they could dial it down.
