Just to addd to the choices we use www.direct-travel.co.uk and get an annual family insurance which covers us for 2 USA trips.

We used Florida Homes and Travel for our travel insurance and car hire.
The travel insurance was a little more than some but my Mum had pre-existing conditions. The car hire was the cheapest I could find.
