do I have to rent a car?

Would it be a PITA to get to MK with my 70 year old mom?

Is it worth it to spend $550 more to stay at the Contemporary where it's easy for mom to get on and off the tram to MK?

I LOVE DTD and I think we would have a lot of fun there.

Are there shuttles in the morning going from the Hilton to MK? Do they run often?

There are shuttles, but i dont know how convenient they are. If you had a car, its not that big of a deal to drive to MK from the Hilton. Now in terms of being "worth it," that depends. To some people, $500 isnt a problem and to some it is a vacation budget buster. Of course the CR is more convenient to MK, but you'll ultimately have to decide what the dollar amount of convenience is.

If you stay at the Hilton, you will want to rent a car, especially because of your mom. The shuttles are okay- pretty good for offsite stayin' actually. BUT, they do not run as many shuttles as Disney Transportation, therefor, they tend to be unpredictable and crowded at times. The standing and waiting to return back to the hotel from mk can be long and painful. Alternatively, why not consider staying at a mod? While I'm not a huge fan of Disney transportation, it is certainly more convenient and more reliable than an offsite hotel shuttle

We did a split stay in 2007 between DTD Hilton and an on property resort. We found the shuttles to be frequent at the Hilton, and if memory serves, they were very convenient. We never waited more than a few minutes. We found the busses to be better timed than Disney transportation.
DTD was t a brisk 5-7 minute walk for us, but with your mom, it might be be longer and could be the deal breaker. Hope this helps.
