I'm sure all the creative folks here can give you better details, but I can tell you what I have done ...
I have bought plain tshirts in different places - AC Moore, Target,
Walmart, and recently because I needed a ton of shirts, I ordered from jiffyshirts.com and got them for $1.55 each.
As far as the image, I always just use something in a .jpg format. I have used photos and clipart. But since finding this amazing board, I have used designs from the DISigners here. If you scroll through the DISigners threads you may find something you like, or you could request something from the great folks that do the Designs if you want it personalized. Just save the image to your computer (right click on it and select 'save as') and then it is yours to use on your tees.
I usually just use Word and do 'insert from file' to get the image ready for printing. Buy tshirt transfer paper at any store that sells printer paper and stuff. I have gotten it at Target, CVS, I think AC Moore, Staples. You just print the image onto the transfer paper just as you would regular paper. Read the directions on the transfer paper to see if it requires a reverse image (some do, some don't). Once the image is on the paper, follow the directions on the paper package to iron it on to the tshirt. It is quite easy!
