How do I word a room request?

I will be leaving for 9 days at the Polynesian starting Saturday August 9th, I put in a room request when I made my reservations a few months age, now I want to follow up on this.
I know from reading these boards that people fax their requests directly to the hotel about 4 days before arriving, but how have you worded these requests? Who do you send them in care of? Also to save time if someone out there has the Poly's fax number I would greatly appreciate it.

Personally, I think the sample letter is somewhat wordy. I would be much briefer -- dates of your vacation and prioritized list of requests, because they may not be able to give you everything you want.

Just for fun, I downloaded a few items of Disney clip art and pasted them onto my request.
