I made it in Pages for Mac. There is a table you can insert into a blank canvas document. Then you can simply play with the fonts, colors and graphics until they are what you like. For this table I took out the traditional lined table and placed instead dots. And then I also alternated the colors on each row to make it easier to read. For the Mickey graphic I found it on Google images and using Preview cleaned out the white background, same with the Castle. The font was tricky but I believe I used a Font called Blackmoor (it's a similar font that is used in Fantasyland in MK) or Burton's Nightmare. Both can be downloaded from dafonts.com.
I believe you can make something similar in Word, but I don't think it has a gradient feature for it's fill shape tool.
