Aren't the honorary citizen ones only given out at the ToonTown Morning Madness ceremony? (I;m probably totally wrong, but that was the impression I got from everything I've read.)
Anyway, I didn't know there were 1st timer buttons. Good to know- we are WDW vets, but DH is the only one who has been to DL- back before it was DLR, 23 years ago....

You get seperate buttons for MTTM. I forgot what they look like, they are red and they have Mickey on them and say something about MTTM.. The Honorary Citizen ones are black and purple-ish and they say Honorary Citizen.. You can get them randomly from a CM (I got mine from a CM who took mine and my parent's picture before a parade) or you can ask for them at City Hall..
DH and I have all of the buttons except 1st visit and thats because I didn't grab one on his first visit. We have done everything at DL... lol... Engaged over 50th anniversary weekend, honeymoon, birthdays, reunion with our family by choice... you name it we've done it... lol!

When we were there in November, the first thing I did was go to City Hall to get a First Visit button for my younger son...and they were all out and not sure when or if they were going to get any more in.
We did finally get one at the Star Tours ride; we were chatting with the CM at the front door and when we told her it was our son's first visit, she said said we needed to get him a button and we explained the whole City Hall being out issue, so she told us to ask up at the desk inside the ride. We did, and they had one for us. (Actually, they brought us four because they misunderstood my question and we were getting on the ride, so I just took all of them, and then the next day gave the extras to some children in line in front of me at the DTD Monorail station who were talking about it being their first visit.)
My older son got an Honorary Disneyland Citizen button for correctly answering a trivia question asked by a CM over near the Space Mountain exit.
City hall will have all the buttons listed above in others posts, but I know that you can pretty much walk into any of the little shops in DL or DCA and they will have first time buttons.

This is good to know, and I wish the person at City Hall who couldn't give us a first visit button would have told us this fact. He made it sound like it was something they were doing away with.
