Form for Food Allergies from Special Diets?

In a few of the food allergy related posts I've read that people have filled out a form about their allergy and sent it to Disney. The form is then sent to the restaurants where you have reservations so the chef knows about what you can and cannot eat....Some people have noted that the chefs have even called them at home prior to their arrival.

I e-mailed special diets but all they said that to tell the CM when you arrive at the restaurant and they will relay the info to the chef.

Is the form still being used?

It depends on the number of allergies and whether they are common. Its more to give them a heads up somebody particularly difficult is coming by. If you have common allergies its sort of a waste of everyone's time.

They want the form if any individual in your group has 4+ allergies or if any of the allergies are uncommon or if you have some kind of difficult diet (like for PKU). The chefs are well trained in handling top 8 allergies and don't need a heads up for those typically. They've got all sorts of allergy products on hand which meet the needs of many of these though when you combine numerous of the top 8 it still becomes more difficult which is why they want the form for 4+ allergies.
