Filling in visa waiver forms

Can anyone help. My boyfriend has a visa so does he fill in form on plane??
Then the form that u fill in per familly how do I go about this? Myself and my boyfriend have different surnames but live at same address with our 3 year old who has his surname. we also taking his older daughter who lives at a different address with a different surname again........ help!!!!! How do I go about it??
is it one form for me, one for older daughter and one for my boyfriend and little one together??
 
green form is for people without a visa, white form is for those with a visa, i think the cutoms form is for people in the same household and with same surname, otherwise, fill one out each. think that is the case but someone will know.
 

Sorry, didn't read your post properly. It's one green form per person and one white form per family. My wife has a different surname on her passport as it's from a different country but we still put everyone on the one white form.
I'm pretty sure you will be classed as a family group but you will be advised on how to fill them out if you are unsure when you check in if you ask.
 
I have a visa and a daughter with the same surname and lives at the same address with me and my DF, I always fill in the white form for me, the green form for my daughter who does'nt have a visa then a separate customs form (the blue and white one) for me and my partner, this way I know I have all areas covered! I would hate to get to the front of the queue and be sent back for the sake of not filling in a quick form on that awful 9hr flight:scared1: I just want to get to my villa as quick as poss, no mistake will stand in my way!!!!!:hyper: :hyper: :hyper:

Yes it is confusing. To be honest, I wouldn't worry about it until you check in. That's when you're issued with the forms to fill in. Just explain the situation at the check in desk and they will tell you what's required and issue you with the relevant forms.
 
Make sure you fill everything out on both sides too or you'll be sent to the back of the queue.... you've got to be really silly for that to happen though! :rolleyes1 :rolleyes1 :rolleyes1
