You must have tickets to schedule FPs.

Tiering varies by park.

Yes you need tickets. MK does not tier their Fastpasses. The other 3 parks, you will be able to reserve 1 headliner and two "lesser" attractions.

You also need a resort reservation to make them at 60 days. With a valid ticket, you can make them at 30 with no resort.
