There is a little kiosk by the AMC theater at DTD. They can do it there.
To be clear, all they can do is ISSUE you the physical reward card. They do not do the transfering of points. You still need to call the 1-800 number to do that. We were almost "stuck" on our last night there because I waited until we were at DTD to transfer $50. I had told my girls they could each have $25 to spend as they wish at DTD. Well, I already had the CARD, I just needed to transfer the points. The lady at the kiosk said she couldn't do that. All they did was issue the card. The points are between you and Bank One. So, I called the 1-800 number on the back of my reward card (it was about 10:30 PM so I was worried) and someone answered! and they transfered my points immediately.
Another interesting thing is that I "ordered" the reward card online and transfered $30. They sent it to me and I could see online that I had $30 on it. But when I called the 1-800 (as described above) she said my card was not "activated". I couldn't understand how I could have $30 on it but it not be activated. She said that there was a sticker on my reward card when I received it and I should have called that number to activate it. But, for the life of me I don't think there was a sticker on my card. I usually do that right away. I am not sure if it was because I ordered it online????
Plus, the card is in my DH's name, but I am a authorized user or whatever. ANyway, the lady on the phone when I was trying to transfer my points asked if she could speak to my husband to get his permission!!!! Luckily he was right there. So, beware of this situation too.
