1. You must stay onsite at a Disney-operated resort.
2. You must book a package reservation (unless it's a DVC reservation). WDW has available ticketless packages (just room and dining) for annual passholders. You must book through the passholder section of the website, or ask for them specifically when you call. this is a package, not a room only reservation. You'd need to pay it off at 45 days from arrival, and the regular cancellation/change policies for packages are in effect.
3. None of the plans include tip. Only the deluxe plan includes appetizers. $109 per night per adult is the price for the deluxe plan. The regular plan for adults is about $60 per night.
4. Conference discounted rooms aren't the same as booking a package and you can't get a dining plan with those, even if you are staying onsite. Same thing for bookings made through third party broker sites (Travelocity, Orbitz, Expedia, etc)
You can book an AP ticketless package without the AP. You'll have to show it to the front desk within 24 hours of check in. They don't ask for anything at booking.
