I have a question about the dining plan (I'm on deluxe if that makes any difference)...

Are there consistent rules from restaurant to restaurant as to what qualifies as the non-alcoholic drink? Is there a price limit? I'm specifically wondering about the milkshakes/smooothies at various restaurants or, for example, the pressed coffee or fancy juice at Kona.

Also, I know glow cubes do not qualify and I presume the kids drinks with the lightup tinkerbell don't as well. If I want to order that, can I just pay the difference and get the cup?

If we do order items that do not qualify (ie: a beer) do they put those on a separate bill, or do we get one bill at the end that lists the total it would be if we didn't have the plan, and they apply the plan and tell us the balance?

You get one bill showing what the totals would be plus suggested tips based on that total, since you still need to tip based on something, and any extra charges not covered by the dining plan such as alcohol.

You can then pay for the tip and any other charges -- if we have enough we use our Disney Rewards card for tips.

It has nothing to do with whether you are on the basic DDP or the DxDDP. Unfortunatley there is no consistency though. I know we've gotten shakes/smoothies/virgin drinks at Whispering Canyon, Artist Point, Kona, the Wave, California Grill, Flying Fish, Kouzzina, Yatchsman, Le Cellier, Coral Reef, Pepper Market, 50's Prime Time, Sci Fi, and Mama Melrose. There may be a few other places, but those are the ones that I remember off the top of my head. I don't know about the pressed coffee or juices at Kona. Sorry!

I've been refused at San Angel Inn, Tutto Italia, Maya Grill, Olivia's, and Turf Club. I simply asked if I could get anything other than soda, milk, tea, or water and was told no.
 
The dining plan covers alcohol only at the 3 dinner shows, where certain types of alcoholic beverages are included with the meal. (That's excepting the wine and dine plan which guests can add to their package in conjunction with a DDP that includes table service, for an extra $40 per night.)

There's no consistency from restaurant to restaurant. It's up to each individual restaurant what they will allow to be covered by the beverage allotment on DDP. It may be so narrow as to depend on whether or not your server is aware that you can get something other than what is listed in the brochure. Unless it's a dinner show, alcohol will never be covered, and souvenir cups/light-up novelties/glow cube drinks are never covered. (Yes, you can order these if you pay for them separately)
 
In my experience, things like milkshakes/ smoothies can be considered either your non-alcoholic beverage OR it can count as your dessert.
 
While we're on the topic of (non-alcoholic) drinks with DDP...would we get charged for an "extra" drink if say, we had soda with our entrees and then wanted coffee with dessert? I can't see them being that strict about it, but you never know.

Yes you will be charged for the extra beverage. Soda, juice and coffee are 2 seperate beverages and the DDP only includes 1 beverage. However, if you are post-meal coffee drinkers, water does not count as a beverage so you could have water with your entree and coffee after with dessert and not be charged anything extra.

Gotta admit, I'm a little surprised by that. Okay, forgetting about the coffee for moment, and I decide to have soda with my entree...do you get refills or just one glass and they charge for refills?

I've never had an issue getting refills.

Of course though, that does beg the question...what's the difference between another refill of soda and a cup of coffee?
