Diabetes and medical form???

My future DH is a diabetic. He is not on insulin and controls his blood sugar with diet. He will be bringing his monitor and lancets. Do we need to fill out the medical form for him? If so. How do I get one and when do we send it in?

I would recommend that you do use the form. I have low blood sugar and I fill out the form, mainly so that the head server and the server in the dining room are aware that I need to avoid any dishes that are prepared with sugar. They help by finding dishes made without sauces and they often find sugar free desserts.

They can also provide you with a sharps disposer for your cabin.

You can get the form by asking your TA to get it for you, or DCL if you booked directly with them. It can be faxed back and forth and your TA has access to a DCL fax server.

I would say you should get the form back to DCL soon after the final payment date.
