DDP and add-ons

Ok say you're placing your order and sometimes you have that option to add chicken or shrimp to that dish or add bacon to your cheeseburger for like $.75 is this something that you pay oop for the add-on or they include it in the DDP? I'm having a hard time remembering from our last trip. Thank you!
