i have mobility issues from a car accident awhile back and i have a tear in my lower lumbar that can cause some issues with standing still for too long or walking for extended periods of time without a break, what is covered under the DAS pass, how do you apply for it?

There have been changes made to the DAS program and based on what you described you are unlikely to be granted DAS. Disney would recommend using a wheelchair or an ECV for mobility related issues.

Specifically about DAS, it is requested via a video call. The link is on the website and app. However as PP noted, what you describe is likely to be considered a mobility/stamina need and WDW’s recommendation is to use a mobility device such as a wheelchair, ECV, rollator or now they allow a cane-seat. A mobility device allows the individual to sit or change positions as needed.
