Cost of replacing lost AP

I can't find my AP anywhere. I have until February to find it, but in case I don't, what is the cost to replace it and what do I need for proof of identity? Thanks!

I believe it's $10, but you might want to call DL to be sure.

Also (and I don't think this applies to you) we were told when we got our AP's that if they are stolen you can bring in a copy of the police report and they will replace for free. Hope you find them, I misplaced mine awhile back but then found that ultra "safe" place I put them in!
