Changing the name of someone on a package

If we have purchased a Disney ticket/hotel package, and one of the people end up no longer being able to attend, will Disney allow us to contact them to change that person to another? For context, my friend is currently having health issues which may prevent them from ultimately joining us. In the event they couldn't, my roommate would be happy to take their place. However, I know often reservations are nontransferable and am unsure if Disney ever allows for this, or if we'd just flat out lose the package if we had to make alterations.

Guests in the room can change, just the lead guest has to stay the same.
