Can you WALK from Poly to MK???

If not - what forms of transportaiton are there from Poly to MK and MK to Poly?

You can't walk.

But you can catch the Monorail at the Polynesian's Great Ceremonial House and ride it to the front of the Magic Kingdom, or catch the very pleasant little boat at the Polynesian dock!

It would be a pretty long walk to the MK, and I do not think that there is a path all the way to the MK.
The best way to get to the MK from the Poly would be the monorail. From the poly the next stop is the GF then the MK. Returning from the MK it goes to the Contemp and then the TTC and then to the Poly.
There is also a boat that goes from the Poly to the MK, I think it goes from the Poly to the GF and then MK.

You can't walk from Poly to MK! But, they do have the monorail and boats! If you want to walk to MK, then you need to stay at the Contemporary and it will be a quick, easy walk!

I also heard that you can walk to the Grand Floridian from the Polynesian. Is this true? If so exactly how long of a walk would it be?

Yes you can walk from the Polynesian to the GF. If you are at Luau cove it will take less than 10 minutes.
