The dining plans are generally an add on as part of a Magic Your Way package. The exceptions are free dining promotions or DVC members. All require you to stay on property.
No. You must be staying at a Disney-operated resort to be eligible for the dining plan. (There are on-site resorts, like the Swan/Dolphin and Shades of Green, that are not operated by Disney).
I believe you also have to purchase at least a one day ticket as well in order to be able to buy the dining plan (correct me if I wrong but that was the policy when we went last).

If you are staying off site, figure buying an annual pass (your parking will be free saving the daily parking fee) then buy the TIW card. Its either $65 or 75--- It will give you 20% off food and beverages (including alcohol) at a wide range of WDW venues.
Depending on your plans and how many days you are there, it may pay off.
For me, the AP and TIW card are reasons to schedule another trip before they expire.
This plan has saved us a lot of money especially since the TIW card can be used for up to 10 people and we usually have 4 adults and 3 kids in our party.
To get free dining everyone in the room must buy at least 2 day base tickets, but to buy the DDP only one day base tickets are required.
AP holders paying rack rate or using an AP discount, people staying on DVC points, and people using some of the army deals can buy the DDP without buying any park tickets.
