Hi. I don't post much. I'm more of a lurker, but you people are usually so helpful I figured I would ask. I am trying to apply for the Sam Walton Community Scholarship because I am a senior in high school. Anyway, I completed the online portion. There is a checklist of other items I am supposed to mail in, but everytime I click on the link my computer says error. SAM WALTON COMMUNITY SCHOLARSHIP PROGRAM
$1,000 Community Scholarship Guidelines

Eligibility Requirements
• Student must not be a Wal-Mart associate, or be the immediate relative (son/daughter) of a Wal-Mart or Sam’s Club associate. If it is determined that the student or an immediate relative of the student is employed by Wal-Mart Stores, Inc., the applicant will be automatically disqualified.
• Student must be a graduating high school senior enrolling full-time in a 2- or 4-year accredited* college in the United States no later than the fall of 2006. *Accrediting bodies funded are AABC, AARTS, ACICS, MSA, NASC, NCA, NEASC-CIHE, SACS-CC, WASC-Sr., WASC-Jr.
• Student must have a cumulative high school grade point average of at least 2.5, and have taken either the ACT or SAT standardized tests.
• Student must have been a Permanent Legal Resident for at least one year upon applying for this scholarship.
• Students already receiving a full scholarship or students electing to attend one of the military academies will not be eligible to receive this scholarship. Military academies include: US Air Force Academy, US Naval Academy, US Merchant Marine Academy, US Coast Guard Academy, US Military Academy (West Point), etc.
• Applicants who will not be enrolled in the Fall 2006/Spring 2007 academic year are not eligible to receive this scholarship. Students planning on attending co-op during the Fall 2006/Spring 2007 academic year forfeit their eligibility for this scholarship.
• Student is allowed to apply once per year using the store number from their local Sam’s Club or Wal-Mart store, but not both. Multiple applications will result in disqualification.

Awards
• This is a $1,000 scholarship payable over one academic year.
• The award is made payable to the college on behalf of the student and will be mailed by SPA to the applicant by August 1 2006.
• This award is non-renewable and is for the first year of undergraduate study only. It is to be applied to tuition, books, supplies, fees and on-campus room and board only.
• Recipients are required to supply SPA with all requested information and to notify SPA of any changes of address, school enrollment or other relevant information. The Wal-Mart Foundation reserves the right to alter or discontinue this program at any time.
• The scholarships will be applied to educational expenses at accredited, nonprofit two or four-year colleges/universities in the United States (hereafter referred to as either colleges or institutions). A student may transfer from one institution to another and retain the award. Immediate notification must be made to SPA to insure prompt payment.

Winner Selection
• Selection of recipients is based on a combination of financial need, academic record, ACT/SAT test scores, extracurricular activities, and examples of leadership in school and the community.
• The student will not receive the award if they are unable to demonstrate financial need, as calculated by SPA using the financial information provided with the application.
• Selection of recipients is made solely by SPA and in no instance does any officer or employee of Wal-Mart Stores, Inc participate in the selection of recipients.
• All applicants agree to accept the decisions of SPA as final.
• Applicants selected to receive the scholarship will be notified in writing by May 1. (Non-winners will be notified of their status by email by May 1.)

Checking the Status of Your Application
To check the status of your application, return to https://www.scholarshipadministrators.net/ and click the ‘Check Application Status’ link. Enter your Application Identification Number [AIN], first and last name. “Submitted” means your online application has been placed on file; it will not be reviewed and processed until all of your documents have been received. Once reviewed and processed your status will change to “Complete”. Incomplete applications will remain as “Submitted”; it is up to the applicant to ensure a complete application packet is mailed by the postmark deadline.

Responsibilities of Recipients
Recipient responsibilities include enrolling as full-time undergraduates in the fall of the year in which the scholarships are awarded and continuing in school for the entire academic year without interruption, barring illness, emergency, or military service. Scholarship recipients are responsible for delivering their scholarship checks to the proper office at their institution.

Completing the Process:

Please use the checklist below to ensure that you follow all steps correctly.

ALL DOCUMENTS MUST ARRIVE IN THE SAME PACKAGE FOR THE SUBMISSION TO BE CONSIDERED COMPLETE. INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED.

• Obtain and attach a copy of your high school record which includes at least six semesters of grades and all standardized test scores. This does not have to be an official transcript. If your ACT and/or SAT scores do not appear on your record, attach a copy of the scores as received from the College Board and/or ACT.

• Make a copy of your signed 2004 or 2005 federal income tax return that shows your adjusted gross income. [Originals will not be returned.]

• Assemble all documents in the order listed below:
• Cover Page printed from the online application
• This Instruction Page
• High School Record
• ACT and/or SAT scores (if they are not on your high school record)
• A copy of parents or legal guardians most recent Income Tax Form 1040

• Make an extra copy of all your documents and keep for your personal records.

• Mail the completion documents to the address above postmarked by January 20.



CERTIFICATION
I certify, to the best of my knowledge, that the information on this application is complete and accurate. Falsification of any information will cause my disqualification from the scholarship competition.

I understand it is my responsibility to make sure this application is completed and mailed by the required postmark deadline listed on the application. Furthermore, I understand that if my application is not complete, or if I do not submit my application by the postmark deadline, I may be disqualified from the scholarship competition and may not be considered for a scholarship.

I understand these documents may be scanned and agree not to staple, paper clip or otherwise attach documents together

This application, upon receipt, becomes the property of the scholarship sponsor, and of Scholarship Program Administrators, Inc., the administrator of the scholarship program.

I give permission to Wal-Mart/Sam’s Club and the administrator to use my photo and any information obtained for publicity purposes, related strictly to this scholarship.

To comply with the provisions of the Family Educational Rights and Privacy Act of 1974, I hereby give permission for school officials to release my secondary school record and other requested information, if necessary.

Thank you so much. I appreciate it greatly. I hope I get it to. A $1000 is awarded to every county each year and someone from my school got it last year so I am hoping and praying.
