Can someone help me?

A few months back someone posted a website (I think) where we could print out a medical consent form. Does anyone know the website? I have searched hi and lo and can't find it.

I am leaving my kids with my parents for a week while dh and I go on a cruise and sinse my parents live 6hrs away from us I know if anything would happen to one of the kids they would need a consent form for medical treatment. (right?) :confused3 Can you tell I have never left my kids with anyone.LOL

I think you can just hand write a note. Call your doctors office and see what they require.

Just sent you an e-mail with the authorization form that we use. I got it from a website, but I don't know which one. For extra precautions I also get it notarized. Just found out that if you have AAA membership they will notarize for free.

Obviously I did not see the e-mail the other poster sent you, but I agree with what someone else just said -- it doesn't have to be any sort of official "form," it just has to state in clear language to whom you give consent about whom and for what purposes. My wife is a pediatrician, and we leave this sort of letter whenever we go away without our kids, or even when the grandparents (who live 100 miles away) have them for a few days. Make sure to include language that the person/people you identify have your permission to consent for the purposes of medical diagnosis and treatment including x-rays, surgery, medication, etc. The good news is, in the event of a God-forbid EMERGENCY, an emergency room physician or primary care doc doesn't need ANYONE'S permission to perform life-saving or function-saving procedures, so even if you forgot to leave a letter, this would result in inconvenience rather than any real medical threat. They would just attempt to contact you by phone to get consent for treating, for example, an ear infection.
