Can someone give me crash course on the Dining Plan?

As we're starting to plan which restaurants we want to hit up when we're in WDW, I want to make sure that I'm understanding the dining plan.

1. It sounds like most restaurants are on the plan, but some require 2 credits or dont' participate. Is this posted clearly outside the restaurant at the park?

2. Okay, so I've looked at a few menus, how do you order from these on the dining plan? What is included. I know that obviously an entree is included and no alcoholic beverages for adults, but what about the kids specialty drinks? Are those included in the meal plan? Is Dessert? (For table service... but really have the same question on QS too)

3. My youngest is 2 and can't be on the plan so we'll be sharing with him. Are the restaurants pretty accommodating in providing an extra plate to share with him? Or should we plan on/is it required to purchase a meal for him? Or just a drink?
