Birthday invite request please?

I know this isn't normally the correct chain of command on this board. But I have limited time in order to get these printed and to my daughter's class before school ends.

My daughter is turning 4 and I would like to mimic the following invitation and incorporate her picture.

I would like the top photo to include my daughter's photo in the space where the photo is.

I've included three of my daughter's photos and you can use whichever one works best. (I hope you can take out the ugly living room background)

As far as the invitation is concerned, it doesn't have to be the exact designs, just something like it. I would like the same colors used.

The wording I want is :

It's time for your check up!
PLEASE JOIN US
Doc MCJerrica
is turning 4!

Your appointment date is June 23rd
Check in time is 1pm
Check out time is 4pm
(ADDRESS GOES HERE. IF SOMEONE WILL PLEASE CREATE THIS INVITE FOR ME, I WILL MESSAGE YOU MY ADDRESS)
RSVP by call or text to Bri at (will include phone number in message)

Basically its the same wording as the photo above, I just incorporated my own information

Can someone please help? It doesn't have to be an "Exact" replica. That was just the general look I was going for. My daughter's last week of school is this coming week, and I really need to get her invitations out to all of her classmates or she will be devastated.
