Baptism design for guest book

Hello!

I am looking to make a notebook to use as a guestbook for my son's Baptism, which is in three weeks. If someone could make a design for the cover featuring the following, I would greatly appreciate your time and help!

(White background and blue letters)
Dylan's Holy Baptism
July 8th, 2012
(With an ornate cross in blue or black)​

If you think of anything else to add that would be cute, feel free! I'm not that creative as you can probably tell!
