A Fish Extender is something that is hung from the fish-shaped mail clip that is next to each stateroom door. Many people use custom pocket organizers like this:

However, I have also seen tote bags, gift bags and even things like treasure chest. As long as it can be hung from the fish, all is good.
For the Fish Extender exchange, this can either be done in one big group for everybody signed up or divided into smaller groups. The second option is what we are going for with this cruise, but people have the option to sign up for more than one group.
If you are signed up, you give something to each of the cabins in your group or groups. How you want to do this is up to you. Some people just do a gift per cabin. Others give a gift per person. Yet again others do a gift for the cabin and individual gifts for the kids. What you give is also totally up to you. Things that I have received over the years are candy, glow sticks, temporary tattoos, Cruise Survival Kits with the likes of cable ties, band aids, tissues, highlighters, safety pins, etc., things from people's local areas (pins, post cards, state quarters, etc.), things that are personalized for the cruise like magnets, silicone bands, pins, tote bags, mouse mats, drinking cups, etc. and there are usually some people that make their own gifts. It does not have to be expensive or elaborate, it is just meant to be a bit of fun. I know a lot of people get stuff from companies like Oriental Trading.
I always get a big buzz when I come back to the cabin and find something in my Fish Extender.

I got so excited when I spotted the Dream Dooney and Bourke Cross Body Bag for less than $100, until I noticed that to ship this to the UK would be $60. I don't know how they can justify those shipping charges.
