Anyone have DLP shares through Credit Agricole?

Ages ago I bought shares in EuroDisney SCA via a broker which were then put into my name (e.g. I have the certificate and everything, rather than the broker holding on to them on my behalf).

When they stopped being traded in London, they were transferred to Credit Agricole Investor Services (who are the company's official registrar). When I was notified of this, they sent me all the paperwork, plus a form to fill in in case I ever wanted to buy any more shares. Everything at this point was fine.

With the price per shares being so low, my Shareholders Club card about to run out and my number of shares being just below the 2,000 limit, I thought now would be an ideal time to dig out this form and take a look at it.

Except I can't make head nor tail of it. There's some bits I can take a stab at filling in, but I'm lost on others, plus I can't see where I send it off to or how I actually pay for the shares (after all, I don't know what the price will come to after they add taxes and fees).

Has anyone else got this form or (even better) used it before who could help me out?
