Overview

Hiatal hernia is a condition in which a portion of the stomach protrudes upward into the chest, through an opening in the diaphragm. The diaphragm is the sheet of muscle that separates the chest from the abdomen. It is used in breathing.

Causes

The cause is unknown, but hiatal hernias may be the result of a weakening of the supporting tissue. Increasing age, obesity, and smoking are known risk factors in adults.

Children with this condition are usually born with it (congenital). It is often associated with gastroesophageal reflux in infants.

Hiatal hernias are very common, especially in people over 50 years old. This condition may cause reflux (backflow) of gastric acid from the stomach into the esophagus.
Symptoms

* Chest pain
* Heartburn, worse when bending over or lying down
* Swallowing difficulty

A hiatal hernia by itself rarely causes symptoms -- pain and discomfort are usually due to the reflux of gastric acid, air, or bile. Reflux happens more easily when there is a hiatal hernia, although a hiatal hernia is not the only cause of reflux.
Tests & diagnosis

* Barium swallow x-ray
* Esophagogastroduodenoscopy (EGD)

Treatment

The goals of treatment are to relieve symptoms and prevent complications.

Reducing the backflow of stomach contents into the esophagus (gastroesophageal reflux) will relieve pain. Medications that neutralize stomach acidity, decrease acid production, or strengthen the lower esophageal sphincter (the muscle that prevents acid from backing up into the esphagus) may be prescribed.

Other measures to reduce symptoms include:

* Avoiding large or heavy meals
* Not lying down or bending over immediately after a meal
* Reducing weight and not smoking

If these measures failure to control the symptoms, or complications appear, surgical repair of the hernia may be necessary.
Prognosis

Most symptoms are alleviated with treatment.
Complications

* Pulmonary (lung) aspiration
* Slow bleeding and iron deficiency anemia (due to a large hernia)
* Strangulation (closing off) of the hernia

The most common complication associated with hiatal hernias (according to my PCP and GI) is severe abdominal pain. Mainly because it can be so sudden and so hard to alleviate the pain. Smaller ones dont usually cause problems but larger ones do and if he has had one for years, his drinking, smoking, etc has probbaly cause it to get worse.

Mine is aggravated by one of my meds I take. Its not severe but it can cause abdominal pain. I had an upper GI done 2 weeks ago and we discovered that it has grown a little bit and probably what has been causing my abdominal pain and wait loss. We're treating the symptoms now, But I have ended up in the ER with the pain associated with it.

Yes a hiatal hernia can be caused by something that someone does. it can be caused by inherited muscle weakness in the area but it can be aggravated by other things and can be caused by other things as well. One of the ways to control symptoms is to avoid alcohol...there's a reason for that. and his drinking probbaly aggravated it.
 

I just looked up Charlie & was surprised that he's only 45. I thought he was around 50. I liked him on Spin City. I hope he recovers and that this incident serves as a wakeup call.

That's the last count I had. His oldest was 25 in September (when she got married), and it bugs the heck out of me that the "stars" he spends time with are often younger than that! Just so gross.

If he hasn't woken up after his entire LIFE...I'm not sure that this will do the trick. His rock bottom is a whole different definition of what would be MY rock bottom...he's already surpassed what I imagine would be my version of it 100 times.

Have you never seen his career defining performance in Scary Movie 3? It was like we were not even watching an actor, he so completely enveloped himself in that role. Quite simply, I consider him to be a national treasure, a true gift from Hollywood - porn stars, cocaine filled briefcases, and all.

Yes. One is legal, and the other is not. In California, there is an industry imposed "standard" that requires testing of the "stars" (DH thinks it might be every month, but he can't recall). Nobody will work with anyone who is not currrent on their testing. DH watched some documentary.
