Another passport for kids question

This is probably a stupid one. We got our kids passports in the mail yesterday. On the passport instructions it says not valid until signed(something to that effect). Does a parent sign it? With their name or the kids? Or does the kid actually sign it?
 
wife said she signed the childs name then printed by mother then signed her name

ie childs name ... by mother or father .... sign your name
 
I called the Passport agency and was told:

Sign the child's name, print the word "by" and then sign my name, then in parentheses put the relationship

For example:

John Smith by Mary Smith (mother)
The names must be signed and not printed she said.
 

Oh and the lady at the Passport Agency I talked to also said to do it for all my children, even my 11 year old son. I would just call and double check if you have any questions. She told me to do it if they were under 14. :confused3
 
When my DW & I took our DD's (11 & 6) to file the applications, we were told by the clerk to have them sign their own and we both had to sign them too. We applied 19 days ago and are hoping they get here soon.
 
When we went to get our passports a month ago, the clerk said that we had to sign for the kids. DH signed for one DD, and I signed for the other DD. Then the clerk told us that we had to remember who signed for who because that is how it should be on the passport. Of course, if he had told us that before we signed the application, I would have signed for both girls! Now that we have our passports, we can't remember who signed which girls names!

Guess I'll have to call!
 
Here's what I found in the FAQs on the Passport web site.

My child is too young to sign his/her own passport. How do I sign my child's passport?

In the space provided for the signature, the mother or father must print the child's name and sign their own name. Then, in parenthesis by the parent's name, write the word (mother) or (father) so we know who signed for the child.
