I'm going to admit that I haven't read through the entire thread (I know, I know). 
My kids love to do Mad Libs whenever we go on a long car trip, especially to WDW. There is even a Mad Libs entitled Mad Libs on the Road.
We also have a travel "Uno" card game. You can find a bunch of travel-size games at Walmart in the board game aisle.
I have also downloaded several free stories onto their iPods for them. Librivox is great for this and they have some of the "classics" like Anne of Green Gables and Tom Sawyer.
If your car has a CD player, you can also buy audiobooks (CDs) or rent them from your local library. I have my eyes on some of the Harry Potter books-on-CD for our next trip in November.
