Oh Boy my little one(7) is going for a short visit to WDW with his nana. What kind of forms does my mom need for him to go with out his parents??
 
My kids often travel with my parents. I would suggest providing them with a notarized letter that says he has your permission to travel with them. I would also send his health insurance card and a letter authorizing treatment, just in case something unforseen happens.
 
check with your pediatritions office or better yet your insurance carrier.
they may only require the letter of consent for treatment that Michelemouse mentioned, but they may require that it be signed by a witness and a notery. (sp?)

All in all, not a big deal, but very handy if your DS gets there and needs a simple medical treatment.

We had first hand experience with this in June. My sister and bil were traveling with their 2 year old granddaughter. They had a signed statement from their son and dil giving permission for medical care. They also had his insurance card. They took her to a walk in clinic but were told they couldn't treat her because the permission statement wasn't noterized. They had to take her to the ER in Celebration and were told they would treat her but they couldn't charge it to the father's insurance without the baby's SS#. My bil had to be responsible for the bill until her could furnish the hospital with that SS#. So the lesson learned is that you need the signed permission statement that includes SS# and the child's SS#. This is for medical treatment in Florida. Other states may differ.
We have flown many times with a niece or nephew and never had to show ID for either of them to travel.
