I booked the extreme getaway for our Castaway Cay day next month. When I made the reservation I did not see an option to pay. did I just not look in the right place, or do you not pay until it actually happens in case of bad weather?

Excursions are placed on your onboard account once the cruise starts.

You'll see your reservation for the excursion online, that ensures you do have it reserved. You pay for it after you actually go on the excursion when you settle your account at the end of the cruise (along with all other onboard charges), that way if it is canceled you won't be billed for it.

Im guessing that if it were on your reservation, and you were to pay in full, then DCL would have the (albeit rare) problem of having to refund you if the excursion didn't happen.
