Companion Restroom locations

I could have sworn there was a section in Sue's FAQ thread that lists all the companion restrooms. But I'm not finding it this morning.

I can only think of 2 at MK right off the top of my head, there's one at the restooms by Gaston's and one near Space Mountain. Where are the others at the MK?

We use them in POC shop, inside Cosmic Rays, pretty sure at Tangled and if you are at CRT one on level with Cinderella.

Do the family rooms have a surface to change diapers on an older child or should I just stick to First Aid?

Many of the Companion Restrooms don't have a changing table and most of the changing tables are 44 inches or shorter.

The thread I linked in an earlier post had the measurements. A few that previously had changing table don't now (some of the ones in Future World at Epcot - I need to do some editing of the thread).
