We made them big enough for everyone and screen printed a custom Pirate/Disney/DCL logo for our cruise.

I just found some Halloween scar and stitches temporary tattoos on clearance at Charlotte rouse for 99 cents a pack. Kind of a funny thing to have there but didn't seem like a bad idea for pirate night.
A deck of cards maybe?

I went to Cracker Barrel online and ordered the peg game(2.99 each) that they have on the tables they are good for Boys of girls age 8 and up (so I buy them a dozen at a time)

I made these for teenage boys - it is a set of dice and a sheet with dice games and a deck of cards. For a dice cup I got the containers that you can buy Oreos in at the gas station - covered them and made them cute. They also got flashlights.

I would stay away from anything like eye patch and squirt guns. Teenage boys would be too old for this. Go with something like a key fob that they can use once they start driving.

The teen boys in our group were 13. We gave them bandanas and eye patches (which they did use, one was our nephew so I know). We also gave them packs of Disney trading cards. DH got them at TRU.
