We had similar experiences on the Dream earlier this month.
I requested booster seat when I booked for our cruise. There was no booster seat for him since day one. I asked our assistant server on both first and second night. He brought it out for my son on the first night and completely forgot about it on the 2nd. So I didn't bother to ask again for the rest of our cruise. I agreed with PP.
