Do you Still Have to Fill out a Medical Form?
We cruised 5 years ago on the Magic and will be sailing this Christmas on the Fantasy. Last time we needed to fill out a medical form. Do they still require the medical form along with the Signature Form upon embarking?

I completed my on-line check in and it did not say anything about the Medical form. When do you get it for it to be filled out?

Wait a minute....getting into terminology issues here.
The old "Medical form" required a doc's signature and was needed if you required an HA stateroom or other special equipment or services. This has been renamed the "Special Services" form and is available on line at the DCL website. It needs to be completed ONLY if you are requesting special services or equipment--for instance, if you needed a gallon of distilled water for a CPAP machine. The health questionaire is part of the check in at the terminal. It asks about fever, gastrointestinal symptoms, cold symptoms, etc. within 3 days of the cruise. One per stateroom is required. |

Ok, great - thanks for the clarification. 5 years ago we needed to have the "medical form" signed by the doctor (even though no special services were needed).
I did see at online check-in the "special services" form and that does not apply to us, so I guess we will just answer the questions at the terminal, and no medical form is necessary. Thank you for all the great information on this trusted site! Getting excited!! Thanks, Sharon |
