You typically walk into sickbay and simply state “Computer, activate the EMH.” At that point the EMH comes online via various holographic emitters which are placed throughout sickbay. EMHs have also been known to be transferable to the Holodeck, or can even roam freely via a Mobile Emitter...

Seems to me that with the change of rules, more Fastpasses will not get used as people miss their window while getting caught-up in other park activities. Perhaps this is by design? I certainly enjoyed the convenience of open return times, and am disappointed that Disney changed their rules...
