Is it just one page with a front and back cover?

I could probably throw something together with the next 2-3 weeks (if not sooner)...no guarantees though.

What overall size are you thinking? Something like each page being 7 inches tall and 5 inches across, such that if you took a piece of paper that was 7 tall x 10 across, you could print the ID and checklist on one side and the front and back cover on the the other side and then just fold it in half??

Yes, it looks to me like the original was just one page with a front and back cover. If it's easier to do each planet on a different page, that would be fine too. And the size sounds perfect.

We don't head to the World until the end of October so there is absolutely no hurry. DD loves Star Tours and I thought she might like to check off her "missions'. Thanks for looking at this for us.

I'm looking for the Ariel princess in the third style you posted [brownish-gold background]....Do you that image available?

I'm hoping to frame each princess individually and put in my daughter's room. I couldn't find the Ariel in that style.
