Order of Ceremony w/ Rev. Jack Day

Does anyone have the order of their ceremony that they used w/ Jack Day? I have asked him to email me a sample but he hasn't gotten back to me yet. We'd like to get the programs done this weekend but can't without knowing how the ceremony is going to go.
