I see that for a childs table service meal it includes an appetizer. For restaurants that dont show appetizers listed (most say soup or salad, some don't show anything) do they get to choose any appetizer on the menu????

The soup or salad IS the appetizer.

I realize that it is soup or salad at the places listing it. Some places are not showing any appetizers listed under the child's menu and I was wondering how that worked where kids get an appetizer?

At all the TS places we have eaten at they had 2 or 3 appetizers specifically for the kids menu. They may not be listed where you a looking, but the kids have never been able to order an appetizer of the adult menu. What are the specific places you are seeing that don't have an appetizer listed for kids? I may be able to tell you what our kids were offered so you have a better idea. :)

2 of the places I don't see Child's appetizers listed are:
Big River Grille & Brewing Works
Olivia's Cafe

Well...of course we have never eaten at either! So I can't help you with specifics. Sorry! Maybe someone who has eaten there can help you. :)

If it's not on the child's menu, they do not get to select an appetizer from the regular menu. Most, if not all, the children's menus are assembled as three "courses." The first course is usually something like a small cup of soup, raw vegetables, or a small green salad. That is considered the child's appetizer for the purposes of the dining plan.
On Olivia's children's menu at
http://allears.net/menu/menu_okk.htm
the appetizer is either chicken noodle soup OR a garden salad.
Big River Grill is not a Disney restaurant, so its menu is not set up in the typical Disney fashion. What has been reported about non-Disney restaurants is that the child just does not get an appetizer if there isn't one listed on the child's menu, because they don't get to pick one off the adult menu. You'd have to ask there to know for sure.

The child menus are ridiculously limited..... appetizers - carrot sticks? who goes to a restaurant to get carrot sticks? Ask for other items... I found most restaurants willing to serve items not on the menu without any charge. If there were a charge, I'd just pay for his meal, not use DDP.... but there never was. Usually brought strawberries instead of fruit cup, fries whether on menu or not, his favorite vegetables - peas, mickey mouse pasta, hot dog, etc.....
