DTD: Wolfgang Puck Express. It is not only one of the best places for a QS meal in Disney, but it is one of the most cost-effective. You can easily spend $20 - $30 there for a meal. They have salads, pizza, pasta, sandwiches - everything really!
AK: Flame Tree Barbeque - food that you won't find in many other places. It's good food as well!
DHS: Pizza Planet - I'm partial because I love Toy Story, but the atmosphere is fun (especially if you have kids!).
Epcot: QS restaurant in China - I love their orange chicken! If it's a hot day out, it may not be the best place to eat because there's no air conditioned seating, but the eating area generally stays cool.
MK: Toss up between Pecos Bills and Cosmic Rays - Cosmic Rays has many different options, so if your party can't agree on what to eat, go there. Pecos Bills has more of a limited menu compared to Cosmic Rays, but I like the atmosphere much better.

DTD - Wolfgang Pucks is HANDS DOWN the best QS credit for the value. Great food, great value!
MK - Columbia Harbour House - yummy food, great upstairs *secret* seating (it's massive upstairs!)
Ep - Sunshine Seasons - great variety (gotta love the salmon - yes salmon! for QS)
HS - Toy Story Pizza Planet - I'm the only one in my family that loves this place for the food :) I like the veggie pizza (the crust is soooo good and has whole grains you can see). Ok, the rest of the family likes it just fine but where the heck can you eat QS in HS anyway?????
AK - Flame Tree BBQ - I LOVE bbq ribs - yum!!! Great value for the QS credit. The outdoor seating is pretty nice but seriously watch out for the birds.

I agree, but I think Pecos Bill's also wins my vote in terms of consistency of the quality of their food.

DTD - Wolfgang Pucks Express - This is the one in the marketplace - NOT the one connected to the Wolfgang Puck cafe. The menus are not the same. They have excellent fresh pasta and crème brulee at the one in the marketplace. The entree is made when you order it and they bring the food to you and serve it on real plates with real forks. (expect to tip though since they are serving you)
MK - Either the 1/2 Chicken and Ribs combo at Cosmic Ray's (this is enough for two people) or Main Street Bakery. At Main Street you can get the strawberry and Cream Smoothie or frozen latte as a beverage and it is killer. They have decent desserts and really nice sandwiches. I had a Tomatoes and fresh mozzarella on focaccia and really enjoyed it. The breakfast here is also very good.
HS - Starring Roles hand down for me. They have a stacked black forest ham on focaccia type bread that comes with gourmet chips (Miss Vickie’s). You can get a latte (made with real coffee not nescafe) as your beverage and the cupcakes for dessert are huge and tasty. Have you seen the photos of the Butterfinger cupcake?
I think the Commissary is awful (it was good at one time). I really don't like the pizza at pizza planet, but it's hard to get good pizza these days thanks to Dominions.
Epcot - There are a lot of excellent Options here. Morocco and Norway are nice, but my favorite is Sunshine Seasons. The desserts are quality, they have a nice drink selection (vitamin water, smart water, energy drinks, and powerade for example) and they also have many food options. I had the Smoked Ham and Salami on Asiago Ciabatta with fresh grated parmesan and it is huge and tasty (I had this twice). I also tried the seared tuna salad as well as the grilled beet salad with goat cheese and both were excellent for 1CS. I was VERY disappointed with the quality of meat at the Asian station so I would not recommend that. The chicken was dark meat with a lot of cartilage and fat.
AK - Flame Tree BBQ – the ribs are decent and the beans were outstanding. Not many dessert or drink options, but the best CS option at AK.
Also depending on what hotel you are at most of the food courts have really good food and a nice selection.
