Anyone have a 2009 Dining Plan Spreadsheet that you wouldn't mind sharing?

I used to have a great Excel Spreadsheet that somebody shared on here.... it let you put in the name of the restaurant and the cost of each persons drink, entree and dessert, then it figured the Tax and Tip and gave you your Total Cost. I loved it but can't find it anywhere now. Please help!
