For example, if I'm not very hungry and they have brushcetta as an appetizer and just want that for dinner, would they let that count as my entree?

Possibly. That's the best overall answer you can get. This is not a policy prohibition like switching your dessert for an appetizer, so it's up to each individual restaurant. It can probably be stated with some accuracy that if you are allowed to do it, then you can order only one appetizer in place of the entree. Don't know what they'd do if you wanted to order an appetizer "for two" as an entree for one person.
