1. Yes. It's called the Deluxe Dining Plan.
1A. - Not really. You have to stay on property to qualify and you must book it as part of your resort package.
2. YOu pay for it like you pay for your vacation.
You should really read this:

You cannot add a dining plan to a room-only reservation. It must be a Magic Your Way package.
You are not alloted meal credits per day. They're allotted per night you stay at your resort. The regular dining plan is one TS, one CS and one snack credit per person per night. The deluxe plan is 3 meal credits, TS or CS, and two snacks per night. There is no plan that allots two TS meal credits per night.
If you convert your room-only reservation to a package, you must buy park tickets with it. That is the only way you can get a dining plan. You must also pay a $200 deposit and have the balance paid in full no later than 45 days before arrival.
You may wish to read the sticky thread "Everything you always wanted to know" at the top of this thread for information about the Disney dining plans.

There are two exceptions to the MYW plan...if you are staying DVC or have an annual pass you can get the DDP's without the tickets. It is still called a package but you don't have to buy tickets.
