Luggage Tags?

Last year when my family and I thought we would be going to Disney World in October we had saw a design on here for luggage tags. They were made with the paint sample mickey heads from Home Depot. I forgot how the person did it though and I am wondering if anyone can help me out with finding the link to this or giving me some insight! TIA for any help!

I used the green Mickey heads and found little clear plastic holders in Walmart.I found them near the luggage. I slid them in and they fit perfectly:)
