Help Wanted -- Iron-ons

Sorry if this has been asked before (I can't get the search thing to work properly!)....

I've seen other's pics where they had created their own iron-ons to make really fun personalized disney-themed t-shirts.

I'd like to attempt this, but:

1. I don't know where to look,
2. I don't know what kind of supplies I need, and
3. I don't know how to do it.

Please help! I need something that is fairly idiot-proof!
