If someoen says that yu may have a design, do you just save it to your computer? Then print it out on a t-shirt transfer?

Either. For white tshirts you may use "light" or "dark" transfer paper. For darker tshirts you use "dark" transfer paper. If you look at some of the threads where people have posted pictures of their tshirts you'll be able to see what they look like.
