1. Adventurers Club. Kungaloosh!
2. Gah. I have many. Here are my choices for each park: MK = Columbia Harbor House. Epcot = Tangierine Cafe. Disney-MGM: Backlot Express. AK = Tusker House.
3. Epcot.
4. I haven't seen the Pirate and Princess fireworks, but I heard they are fantastic. I think I'll go with Wishes.
5. Chef's Table, Victoria & Albert's.
6. Beach Club Villas. AKA "home".
7. Does Streetmosphere count? If not, Festival of the Ling King.
8. Goofy.
9. 50's Prime Time Cafe.
10. I think the hotel restaurants count towards "fine dining" for the most part, if they're not buffets. I enjoy California Grill, Jiko, Artist Point to name a few.
11. Port Orleans Riverside. I first stayed there when it was Dixie Landings, loved it ever since. (Only stayed in Alligator Bayou.)
12. Pop Century. Single bus stop, great theming, decent food court.
