Consumer Expression (scroll down-no banner) has a lengthy but painless survey and "enter to win vacation sweepstakes" for 50 pts.
Motorola has a 15-25 pts "enter to win a free phone" (I think)contest, but this didn't work for me and my ancient PC. Maybe it'll work for you.
